Four Seasons Health Care, Inc. earned an overall rating of 1 out of 5 for the first quarter of 2026, data from the Centers for Medicare and Medicaid Services (CMS) shows. This placed the facility below the North Dakota nursing home average rating of 3.3 for the same period.

The facility had a capacity of 33 beds, 36 fewer than the state average across North Dakota as of the first quarter of 2026. The center provided care to an average of 24.7 residents throughout the quarter.

As of the first quarter of 2026, Four Seasons Health Care, Inc. had incurred $299,184 in four fines and received four total penalties.

U.S. Census Bureau estimates indicate that the nation’s older adult population is increasing as younger demographics shrink. Between 2020 and 2024, Americans age 65 and up grew by 13%, while those under 18 declined by 1.7%. The country’s older population has risen sharply over the last century, jumping from 4.9 million in 1920 to more than 61 million as of 2024.

A 2021 study estimates that nearly half of this group—about 28 million people—will need long-term services or support, with projections indicating that over a third could eventually require nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education organization. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
